Architectural Review Committee (ARC) Request Form
This request, if approved must be completed as submitted, and comply with all recorded Covenants Architectural Review Committee (ARC) Request Form
Conditions and Restrictions (CC&Rs) of our community. Work must commence no later than 3
months from the date of approval,otherwise the approval shall be deemed revoked unless the
Owner has applied for and received an extension of time from the ARC.
Please refer to the Park Village Homeowners Association CC&Rs when submitting this request.
Please submit this completed form, a simple footprint drawing of where the work will be located
on your lot in relation to your lot line and home. Please be sure to include measurements, materials
used and specific descriptions of the work to be completed.
If painting, include color sample/ swatch of main body color and trim color.
Please remind your contractor not to make loud noise before 8 am, Building materials should be
stored out of sight when possible. Ask your contractor to be respectful of your neighbors .
Do not block any driveways or mailboxes with trucks or supplies.
Be mindful of easements and buried utilities that cross your lot.
Approval from the ARC does not exempt you from any permits or requirements the City of Florence may impose.
